Our athletics program is open to 6th–8th grade students and gives everyone a chance to build skills, make friends, and show school spirit. Whether you’ve played before or want to try something new, there’s a place for you here!
🏅 How to Join a Sport
You must complete all the steps below before you can practice or compete.
1. Register Online
Students and parents need to complete the Final Forms registration.
You only need to register once per school year, but make sure to select all the sports you might want to play.
2. Get a Sports Physical
A sports physical is required before you can participate.
Physicals are good for two years.
Forms needed:
-
CVSD–WIAA Pre-Participation Physical Form (Family Copy)
-
CVSD–WIAA Pre-Participation Physical Form (Doctor Return to School)
You can schedule a physical with:
-

-
Or visit a walk-in clinic:

-
3. Pay Your Fees
-
$20 ASB Fee (once per school year)
-
$15 Sports Participation Fee per sport
-
If you qualify for free or reduced lunch, you may be able to get a sports fee waiver.
Fees must be paid before you can get your jersey or play in a game.
4. Have Medical Insurance
All athletes must have medical insurance listed and verified in Final Forms.
🎓 Academic Eligibility
Athletes must keep up their grades to stay eligible for sports.
-
You need to be passing all classes to play in games or meets.
-
If you’re not passing, you can still practice but can’t compete until your grades improve.
We believe success in the classroom comes first — being a great student makes you a stronger athlete!
